Consent to the Collection and Use of Sensitive Information (Health and Medical Information)

Pursuant to Article 23 of Korea's Personal Information Protection Act (PIPA), UROGYN Woman Clinic (유로진여성의원; the "Clinic") processes health-related information, which constitutes sensitive information, separately from other personal information and only with your separate consent.

Please read the following and decide whether to give your consent.

1. Purposes of Collection and Use

  • Responding to online consultations and cost inquiries, and providing information about medical care
  • Receiving self-assessment and before-and-after photo requests and following up by phone
  • Reviewing previous consultation history when you consult again or visit the Clinic

2. Items Collected

  • Health-related details that you write and submit yourself when requesting a consultation or making an inquiry, such as your health condition, symptoms, and medical history
  • Answers to self-assessment questions
  • Areas of medical interest selected when submitting a request

The Clinic does not separately collect any health information that you have not written and submitted.

3. Retention and Use Period

  • The health-related information you submitted is retained for 5 years from the date of consultation or request and then destroyed without delay. This applies equally to members and non-members.
  • If a consultation leads to medical care, records from the course of care are retained for the periods prescribed by Korea's Medical Service Act.

The information is retained so that your previous consultation history can be reviewed when you consult again or visit the Clinic.

If you withdraw your consent, the health-related information you submitted will be destroyed without delay, except where retention is required by law.

4. Right to Refuse Consent and Consequences of Refusal

You may refuse this consent.

However, if you refuse, you cannot use the following services that require health-related information:

  • Online consultations and cost inquiries
  • Self-assessment and before-and-after photo requests

This consent is optional for membership registration. You may register without consenting and give your consent later when you request a consultation.

Even if you do not consent, you may still use phone consultations and in-person medical care.

5. Processing of Sensitive Information

The Clinic does not use sensitive information for purposes other than those stated above and does not provide it to any third party except as required by law.

Outsourcing of processing to service providers and security measures are governed by the Privacy Policy (Articles 6 and 10).

Before your surgery or procedure

Results and recovery times vary from person to person. Side effects such as swelling, bruising, pain, infection and pigmentation may occur at the treated area. We decide on the method only after a thorough consultation and examination. Please discuss the details with your doctor at your appointment.
